Below you will find all the information you need to become a patient in our office.
SCHEDULING AN APPOINTMENT:
Please include the following information:
* Your name, address, phone number, and insurance information.
* What service you would like, checkup and cleaning, toothache, lost filling, etc.
* What day and time work best for you.
* Let us know if you have current x-rays from a previous dentist.
YOUR FIRST APPOINTMENT:
Prior to your appointment, you may fill out all necessary paperwork by clicking FORMS. You may print the form and bring them to your appointment or email them to us.
If you have current (1 year or less) x rays that you would like to have forwarded to us, please fill out the TRANSFER OF RECORDS form and send it to your previous dentist.
Upon your arrival, Rose will confirm your insurance information and receipt of all necessary forms.
We will discuss your health history, take your blood pressure and review all your medications. Please bring a current list of medications with you. If you have any concerns about your dental health please let Battlefield Modern Dentistry know so that she may address your concerns.
Battlefield Modern Dentistry will then see you for your examination. You will receive a head and neck exam to look for any abnormalities and an oral cancer exam. Battlefield Modern Dentistry will also check the gum tissue for signs of gum disease and the teeth for signs of decay, wear, fractures, and defective restorations.
Generally, you may have your teeth cleaned at your examination visit; however, occasionally it is in the best interest of the patient to reappoint this procedure.
Battlefield Modern Dentistry will discuss her findings and any recommended treatment with you. Please feel free to ask any questions, we love to talk dentistry.
You will be given a written treatment plan that includes our fees and an estimate of your insurance coverage. This is only an estimate of your coverage. (INSURANCE FAQS PAMPHLET) We encourage you to contact your carrier to determine their payment policies. You remain responsible for any fees not paid by your insurance carrier regardless of the estimated coverage. Deductibles and/or co-pays are due at the time of service.
